Job summary

Gentiva is seeking a Hospice Patient Care Manager (RN) in Eau Claire, Wisconsin to lead clinical excellence and inspire compassionate teams. You will oversee patient care activities, coordinate care plans, and ensure compliance across regulatory standards while mentoring staff and supporting performance improvements.

The role requires an active RN license, 3+ years of direct patient care (2+ in hospice/home-based care), and strong leadership and documentation skills.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice or home-based care.
• Knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team and adapt to varied needs.
• Strong interpersonal, organizational, and leadership skills.
• Proficient in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead or facilitate IDG meetings, present updates, coordinate care plans, document minutes.
• Mentor and train associates; support development, performance, and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and company policies.
• Support quality improvement initiatives and operational efficiencies at the location.
