Job summary

Providence at Home with Compassus is seeking a compassionate Hospice Registered Nurse (RN) to provide person-centered care in patients’ homes in Washington state. You will assess needs, deliver professional nursing care, and collaborate with the IDG to plan and direct care.

The role emphasizes end-of-life care, documentation, and teamwork; requires an RN license, BLS, and 1 year hospice/home health experience. A $5,000 sign-on bonus is offered along with a comprehensive benefits package.

Qualifications

• Must hold an active RN license in the state of employment.
• Associate or Bachelor’s degree in Nursing from an accredited program.
• 1 year of Hospice or home health nursing preferred; willingness to learn.

Responsibilities

• Completes assessment of patients and families to determine Hospice needs.
• Writes and initiates the Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care.
• Develops a care plan with patient, family, caregiver, and IDG; set goals based on nursing diagnosis.
• Participates in on-call duties and maintains nursing documentation in EMR.
