Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse to provide person-centered care in patients' homes. You will assess needs, plan and deliver nursing care, and collaborate with the Interdisciplinary Group to direct care plans.

The role requires an RN license and an Associate or Bachelor’s degree in Nursing, with on-call duties and ongoing documentation of care. A $5,000 sign-on bonus is offered along with comprehensive benefits.

Qualifications

• Active RN license in good standing.
• Associates or Bachelors in Nursing from an accredited program.
• Experience in home health, hospice, or palliative care preferred.

Responsibilities

• Completes patient and family assessments to determine hospice needs.
• Writes and initiates a Plan of Care in collaboration with the IDG.
• Administers prescribed medications and treatments per the care plan.
• Participates in on-call duties and maintains documentation of care.
