Job summary

Providence at Home with Compassus in Seattle seeks a Hospice Registered Nurse (RN) to deliver compassionate, person-centered care in patients' homes. You will assess needs, provide professional nursing care, and collaborate with the Interdisciplinary Group to develop and direct individualized care plans.

Requirements include an RN license, nursing degree, and a willingness to obtain BLS within 30 days. Hospice experience is preferred, with on-call duties and adherence to productivity standards.

Qualifications

• Associate or Bachelor’s degree in Nursing.
• Current, unencumbered RN license.
• BLS certification within 30 days of hire.
• Preferred hospice experience (CNA/LPN) considered.

Responsibilities

• Completes assessment of patients and families to determine Hospice needs.
• Writes and initiates the Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan with patient, family, caregiver, and IDG reflecting nursing diagnoses and palliative actions.
• May administer medications and treatments as prescribed by the IDG-approved Plan of Care.
• Participates in on-call duties as defined by on-call procedures.
• Maintains productivity standards as set by organizational guidelines.
