Job summary

Brighton Hospice is seeking a Registered Nurse (RN) to provide comprehensive hospice care to a panel of patients in their homes, Skilled Nursing Facilities, and assisted living communities. The RN will be part of an interdisciplinary team, delivering psychosocial assessments and coordinated care.

Qualifications

Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
Works well within a team environment to collaborate daily with other healthcare professionals.
Able to work independently with day-to-day activities including phone, email, and meetings.

Responsibilities

Provide care and services for a group of patients, visiting them where they reside (home, SNF, assisted living).
Collaborate with the interdisciplinary team on psychosocial assessments and care planning.
Travel across the service area to deliver hospice care and coordinate with patient care teams.
