Job summary

Brighton/Suncrest Hospice is seeking a Resource Nurse (RN) to deliver compassionate, patient‑centered care across home, facility and assisted living settings. You will be part of an interdisciplinary team providing psychosocial assessments and coordinating community resources.

You must hold an active RN license, excel in teamwork and time management, and be willing to travel the service area with reliable transportation while leveraging an electronic medical records system.

Qualifications

• Current RN licensure in good standing in the state(s) of practice
• Ability to work within an interdisciplinary team
• Strong time management and prioritization
• Ability to learn and use an electronic medical records system
• Willingness to travel across the service area with reliable transportation

Responsibilities

• Provide direct care and services to a group of hospice patients in homes or facilities
• Visit patients where they reside, including private homes, SNFs, and assisted living facilities
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Assess psychosocial needs and connect patients and families with community resources
