Job summary

Brighton Hospice seeks a Registered Nurse to provide patient care in homes, SNFs, and assisted living facilities. The RN will be part of an interdisciplinary team, offering psych-social assessments and coordinating care across case management, admissions, or resource roles.

The role requires an active RN license, ability to travel across the service area, and proficiency with EMR systems; collaboration with nurses, social workers, chaplains, and CNA leadership is essential.

Qualifications

• Current RN license in the state(s) of practice.
• Ability to collaborate daily with care team.
• Strong time management to handle visits and meetings.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Provide care and services for a group of patients in their homes or facilities.
• Serve as part of the interdisciplinary team, contributing psych-social assessments.
• Engage in case management, admissions, or resource coordination as needed.
• Document visits and communications accurately in the EMR.
• Travel across the service area to patient locations.
