Job summary

Brighton Hospice in the Milwaukee area is seeking a Registered Nurse to provide compassionate care to patients in home and community settings. You will join an interdisciplinary team and contribute to quality end-of-life care with a focus on visiting patients, assessments, and effective communication.

Responsibilities include traveling across the service area, documenting care, and supporting case management, admissions, or resource roles within the hospice program.

Qualifications

• Current RN licensure in good standing.
• Ability to work within an interdisciplinary team.
• Willingness and ability to travel across the service area with reliable transportation.
• Hospice experience preferred or strong interest in community-based care.
• Strong communication and organizational skills.

Responsibilities

• Provide care and services for patients visiting their homes, SNFs, or assisted living facilities.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work in Case Management, Admissions, or Resource roles as part of the team.
• Document patient care and coordinate visits and plan care accordingly.
