Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse (RN) to visit patients in private homes, SNFs, and assisted living facilities as part of a coordinated care team.

The RN will contribute expert care, psychosocial assessments, and community resource knowledge, while working with nurses, social workers, chaplains, volunteers and CNA leadership to deliver compassionate hospice services.

Qualifications

• Active RN licensure in state(s) of practice.

Responsibilities

• Provide clinical care to a patient group in homes, SNFs, and assisted living facilities.
• Collaborate with an interdisciplinary team including nurses, social workers, chaplains, volunteers and CNA leadership.
• Manage documentation and communications via phone, email and meetings; understand hospice regulations.
