Job summary

Brighton Hospice is seeking an experienced Registered Nurse (RN) to deliver compassionate hospice care to patients in private homes, SNFs, and assisted living facilities. You will join an interdisciplinary team and contribute your clinical expertise in psychosocial assessments and community resources.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work within a diverse interdisciplinary team.
• Strong time management and prioritization for visits and documentation.
• Willingness to drive with reliable transportation across the service area.

Responsibilities

• Provide care and services for a group of patients in homes, SNFs, or assisted living facilities.
• Work as part of the interdisciplinary team to address psychosocial assessments and interventions.
• Support case management, admissions, or resource roles as needed.
• Maintain accurate documentation in the electronic medical records system.
