Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse (RN) to provide compassionate hospice care in patients' homes, skilled nursing facilities, and assisted living communities. You will collaborate with an interdisciplinary team to develop care plans addressing physical, emotional, and spiritual needs, while maintaining lower clinician caseloads and higher staff-to-patient ratios.

The role emphasizes patient-centered care, independent scheduling, EMR proficiency, and reliable transportation

Qualifications

Current Registered Nurse (RN) license in good standing in the state of practice.
A passion for providing exceptional, patient-centered hospice care.
Strong collaboration and communication skills with interdisciplinary care teams.
Ability to work independently while effectively managing your daily schedule.
Excellent time management and organizational skills to balance patient visits, documentation, meetings and urgent needs.
Comfort learning and utilizing electronic medical record (EMR) systems.
Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.

Responsibilities

Provide compassionate, individualized hospice care wherever patients call home—homes, SNFs, or assisted living communities.
