Job summary

Brighton Hospice is seeking a Registered Nurse to provide compassionate home-based care to patients across our service area. You will collaborate with a multidisciplinary team, perform psychosocial assessments, and coordinate resources as needed.

Responsibilities include case management, admissions, or resource duties, with travel to patient locations as required by caseload. A valid RN license and reliable transportation are essential.

Qualifications

• Current RN licensure in good standing in state(s) of practice.
• Works well within a team environment to collaborate daily with nurses, social workers, and other care team members.
• Able to work independently and communicate via phone, email and written notes.
• Willingness to drive with reliable transportation to travel across the service area.

Responsibilities

• Provide care and services for a group of patients, visiting them where they reside (homes, SNFs, assisted living).
• Serve as a contributing member of the interdisciplinary team with psych-social assessments and community resources.
• Roles include Case Management, Admissions, or Resource coordination within hospice care.
