Job summary

Brighton Hospice is seeking a Registered Nurse (RN) to deliver compassionate care to patients in home, skilled nursing facilities, and assisted living settings. You will collaborate with an interdisciplinary team to provide psychosocial assessments and coordinate community resources.

The role may be in Case Management, Admissions, or Resource positions, requiring independent work and strong time management. Travel within the service area is expected with reliable transportation and insurance.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in the practicing state(s).
• Ability to work within an interdisciplinary team including social workers, chaplains, volunteers and CNA leadership.
• Willingness to travel across the service area with reliable transportation and valid license/insurance.

Responsibilities

• Provide care and services to a group of patients in homes, SNFs, or assisted living facilities.
• Collaborate with the interdisciplinary team on psychosocial assessments and community resources.
• Work in Case Management, Admissions, or Resource roles as part of patient care.
