Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to hospice clients in their homes or community health settings in Connecticut. You will assess, plan, and deliver end-of-life care, coordinating with an interdisciplinary team to promote comfort and dignity for patients and support for families.

Qualifications

Education: Associate's or Diploma in Nursing (CT licensure required).
Experience: 1 year nursing experience or 1 year in a specialty.
Licensure: Connecticut license to practice nursing.
Preferred: BSN program graduate.

Responsibilities

Reviews referrals and initiates contact with client and family.
Conducts nursing assessments based on data and observations.
Develops and updates a care plan with the client and interdisciplinary team.
Manages medications and DME per policy and payer guidelines.
Directs LPNs and Home Care Aides in carrying out the care plan.
Provides patient and family teaching on signs, symptoms, and self-care.
