Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to deliver skilled nursing care in patients' homes or community settings. The RN will conduct comprehensive assessments, develop individualized care plans with patient and family, and coordinate with an interdisciplinary team to ensure high-quality, cost-effective care that emphasizes comfort and symptom management.

The role requires CT licensure and at least one year of nursing experience, with preferred BSN and home-care

Qualifications

Education: Graduate of an Associate’s or diploma program in nursing.
Experience: One year nursing experience OR one year nursing experience in a specialty.
Licensure: License to practice in the State of Connecticut.
Preferred: NLN approved BSN program; Medical/surgery or home care experience.

Responsibilities

Reviews referrals and initiates contact with client and family.
Performs complete nursing assessment based on referral data and patient status.
Develops plan of care with client and disciplines; documents goals and resource needs.
Utilizes Hospice levels of care and payer information in care planning.
Directs LPNs and Home Care Aides to implement the plan of care.
Takes follow-up visits to assess disease progress and treatment response; educates and supports patient/family.
