Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care in clients' homes or community settings, delivering high-quality, patient-centered care.

Responsibilities include comprehensive assessments, care planning with the interdisciplinary team, and coordination of services to support end-of-life care. CT licensure required; one year of nursing experience preferred, with NLN BSN preferred for advancement.

Qualifications

• Graduate of an Associate’s degree or diploma program in nursing.
• One year nursing experience OR one year nursing experience in a specialty.
• License to practice in the State of Connecticut.
• NLN-approved BSN program preferred for the role.
• Home health/hospice experience is a plus.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts complete nursing assessment based on referral data and patient status.
• Develops a plan of care with client and disciplines; documents needs, goals, and resources.
• Uses Hospice Levels of care appropriately and follows payer information for care planning.
• Directs and supervises LPNs and Home Care Aides; coordinates all client care services.
• Educates patients and families about signs of illness and end-of-life care; provides bereavement follow-up.
