Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to clients in their homes and community settings in the Mansfield region. You will perform comprehensive nursing assessments, develop care plans with input from the patient and family, and coordinate interdisciplinary care to ensure symptom management and end-of-life support.

The ideal candidate holds an Associate’s degree or nursing diploma, is licensed to practice in Connecticut, and has at least

Qualifications

Graduate of an Associate’s or diploma program in nursing.
One year nursing experience or one year nursing experience in a specialty.
License to practice in the State of Connecticut.
BSN program graduate preferred.
Home care or medical/surgery experience is a plus.

Responsibilities

Provides skilled nursing care to hospice clients in their homes or community settings.
Conducts complete nursing assessments based on referrals and patient status.
Develops and updates plan of care with patient and interdisciplinary team.
Manages medications and treatments, and documents care according to agency standards.
Directs LPNs and Home Care Aides in carrying out the plan of care.
Educates patients and families on signs, symptoms of decline, and care plans.
