Job summary

Hartford HealthCare at Home seeks a Hospice Registered Nurse to join our patient‑centered team. You will deliver skilled nursing care in patients’ homes and coordinate with the interdisciplinary team to support end‑of‑life care with empathy and excellence.

RN candidates should have an Associate degree in nursing, CT licensure, and at least one year of nursing experience (home care or medical/surgical preferred); a NLN‑approved BSN is a plus.

Qualifications

Education: Associate degree in nursing.
Experience: One year nursing experience or one year in a specialty.
Licensure: Connecticut nursing license to practice.
Preferred: NLN-approved BSN program.
Experience: Medical/surgery or home care experience.

Responsibilities

Reviews referrals and initiates contact with client and family.
Performs comprehensive nursing assessment from referral data and patient status.
Develops care plans with client and interdisciplinary team.
Manages medications and DME per protocol.
Directs LPNs and Home Care Aides in plan of care.
Provides client/family education and bereavement support.
Coordinates care with physicians and other providers.
Maintains patient records according to agency standards.
