Job summary

Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, seeks a Hospice Registered Nurse to deliver skilled nursing care in patients' homes. You will review referrals, perform comprehensive assessments, and collaborate with an Interdisciplinary Team to create and adjust care plans ensuring comfort at end of life.

The role requires CT licensure, with ADN or diploma as minimum and BSN preferred; one year of nursing experience or specialty experience is desired.

Qualifications

• Education: Graduate of an Associate’s of diploma program in nursing
• Experience: One year nursing experience OR one year nursing experience in a specialty
• Licensure: License to practice in the State of Connecticut
• Preferred: Education: Graduate of NLN approved BSN program
• Experience: Medical/surgery or home care experience

Responsibilities

• Reviews referrals, and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status.
• Develops a plan of care in collaboration with client and all disciplines based on assessment data and goals.
• Facilitates peaceful death and bereavement follow up through timely documentation.
• Implements and coordinates care with the interdisciplinary team to support patient/family needs.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
