Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to deliver skilled nursing care to patients in their homes across Connecticut. You will assess needs, plan care, and coordinate with an interdisciplinary team to support end-of-life care.

The role requires CT licensure, an associate degree in nursing (BSN preferred), and at least one year of nursing experience or in a hospice specialty. A compassionate, patient-centered approach is essential.

Qualifications

• Graduate of an associate degree or diploma program in nursing.
• One year nursing experience or one year in a specialty.
• Current CT nursing license to practice in CT.

Responsibilities

• Provide skilled nursing care to hospice clients in home or community settings.
• Perform comprehensive nursing assessment and plan of care with client/family.
• Educate patient and family on symptoms, end-of-life care, and medications.
• Coordinate with interdisciplinary team for care planning and bereavement support.
• Supervise LPNs and Home Care Aides as needed.
