Job summary

Hartford HealthCare at Home in Connecticut seeks a Hospice Registered Nurse to provide skilled nursing care in patients' homes and community settings. You will assess needs, plan care, and coordinate with an interdisciplinary team to promote comfort and quality of life.

Minimum requirements include an Associate’s degree in nursing (or diploma) and CT licensure, with at least one year of nursing experience (or in a specialty).

Qualifications

• Graduate of an Associate’s or Diploma in Nursing program.
• Licensed to practice as a Registered Nurse in Connecticut.
• Preferred: BSN and home care experience.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and manages symptoms and end-of-life issues.
• Develops and documents a plan of care with the client and multidisciplinary team.
• Informs and coordinates with physicians regarding patient needs and plan of care.
• Directs/oversees LPNs and home care aides within the plan of care.
• Provides patient and family teaching about signs, symptoms, and bereavement support.
