Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care in patients' homes or community settings. You will perform comprehensive nursing assessments, develop care plans with an interdisciplinary team, and promote comfort and quality of life for hospice clients in Connecticut.

The role requires nursing licensure in Connecticut, with a preference for BSN and home care or medical/surgical experience.

Qualifications

Education: Graduate of an Associate’s or diploma program in nursing.
Experience: One year nursing experience OR one year nursing experience in a specialty.
Licensure: License to practice in the State of Connecticut.
Preferred: NLN approved BSN program; home care or medical/surgery experience.

Responsibilities

Reviews referrals and initiates contact with client and family.
Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status.
Develops a plan of care in collaboration with client and all disciplines based on assessment data, documents needs, discharge planning, goals, and resources.
Facilitates peaceful death and bereavement follow up through timely documentation of bereavement assessment.
Follows payer information to maximize care planning and utilizes Hospice Levels of care.
Directs LPNs and Home Care Aides in carrying out the established plan of care.
Teaches patients and reinforces client/caregiver education; prepares family for signs of death as appropriate.
Coordinates care with physicians and other providers; consults on patient needs and plan of care.
