Job summary

Providence at Home with Compassus is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes within Napa, CA. The role emphasizes community health education and collaboration with the IDG to tailor care plans for individuals and families.

The RN will perform comprehensive assessments, develop Plans of Care, administer prescribed treatments, and provide guidance to patients and caregivers.

Qualifications

• Active RN license in California (or eligible).
• BLS certification through American Heart Association within 30 days of hire.
• Preferred: 1 year of hospice/palliative/home health experience.
• Education to meet licensure and registration requirements in CA.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop, initiate and revise Plans of Care with IDG based on assessments.
• Provide professional nursing care using the nursing process and administer medications.
• Coordinate with IDG and Care Manager to ensure continuity of care at home.
• Document accurately patient condition, care provided and pain/symptom management.
• Communicate changes to IDG and attend IDG meetings as needed.
• Fulfill on-call duties and maintain productivity per guidelines.
• Supervise ancillary staff and support team education.
