Job summary

Chapters Health System in Florida seeks a Per Diem RN to provide compassionate, patient-centered care across hospice settings in Brooksville, New Port Richey, and Wesley Chapel. You will assess patient needs, coordinate the care plan with the IDG, and deliver bereavement support and symptom management with dignity and empathy.

The role requires an active Florida RN license, BLS, and basic proficiency with EMR/EHR systems.

Qualifications

• Current license as RN in the state of employment.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum 1 year nursing experience in hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct nursing care to patients and families as part of the IDG.
• Coordinate the Plan of Care and reflect evolving patient needs.
• Educate patients and families about disease, palliation and safety.
• Report changes in condition to IDG or health professionals.
• Participate in evaluating referrals/admissions for level of care.
• Attend patient deaths and prepare the body for transport.
• Communicate clearly with physicians and staff using SBAR.
• Supervise care provided by LPNs and contract staff when needed.
• During emergencies, may be required to report to a designated location.
