Job summary

Reserve Care is seeking a Registered Nurse to provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team. Implement the Plan of Care and coordinate HP care using the Nursing Process.

You will assess needs, manage pain, communicate with physicians, and provide emotional support to patients and families while participating in on-call, discharge planning, and coordination with volunteer staff.

Qualifications

• Active Ohio RN license.
• Experience in hospice care, home health, oncology and/or pain and symptom management preferred.
• Current CPR certification.
• Must have and maintain a valid driver’s license and auto insurance at a level acceptable to employer.
• Hospice and Palliative Care Certification desired, to be obtained within first 2 years of hire.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients with the transdisciplinary team.
• Coordinate implementation of the Plan of Care using the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Manage pain and symptoms; educate patient and family.
• Act as liaison to physicians and provide patient information as needed.
• Offer emotional support to patients and families; assist with bereavement follow-up.
