Job summary

Reserve Care seeks a Home Care Registered Nurse (RN) in Fairview Park, OH, full-time. You will provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team.

You will coordinate the Plan of Care, assess pain and symptoms, educate families, and maintain comprehensive records. On-call rotations and attendance at care conferences are expected as part of ongoing patient support.

Qualifications

• Provide nursing care to terminally ill patients in home/hospice settings.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Maintain and update care plans and reports per policy.
• Participate in on-call rotations and care conferences.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team.
• Coordinate the implementation of the Plan of Care for each patient.
• Assess pain and symptoms and assist patients and families in management.
• Liaise with physicians and other providers as needed.
• Deliver health teaching to patients and families.
• Maintain required plans, reports and records.
• Participate in discharge planning and bereavement follow-up.
• Assist with volunteer coordination and community outreach.
