Reserve Care in Ohio is seeking an experienced Registered Nurse to provide prescribed medical treatment and personal care to terminally ill patients, collaborating with a transdisciplinary team to implement the Plan of Care.

You will assess needs, manage pain and symptoms, educate families, participate in care conferences, on-call rotations, and discharge planning, while coordinating with physicians and volunteers as appropriate.

Formación

• Licensed RN in the State of Ohio.
• CPR certification is current.
• Must have and maintain a valid driver’s license and auto insurance at a level acceptable to RC and/or RC’s insurer.
• Hospice/palliative care certification desired; will be required within first 2 years of hire.

Responsabilidades

• Provide prescribed medical treatment and personal care services to terminally ill patients in collaboration with the transdisciplinary team.
• Coordinate the implementation of the Plan of Care for each patient utilizing the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of the patient and family/caregivers.
• Monitor and manage pain and symptoms; educate patient and family on care and comfort measures.
• Liaise with patient physicians and coordinate communications as needed.
• Participate in care conferences and discharge planning with other team members.
• Provide emotional support to patients and families; coordinate with volunteers as appropriate.
• Maintain and complete all necessary plans and reports related to hospice care; participate in on-call rotation.
