Job summary

Reserve Care seeks a PRN Registered Nurse (RN) for the Westlake Inpatient Facility to provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the care team, coordinating the Plan of Care.

Responsibilities include assessing physical, psychosocial, and spiritual needs, monitoring pain and symptoms, on-call participation, discharge planning, and contributing to care conferences.

Qualifications

• Licensed RN in the State of Ohio.
• Experience preferred in hospice care, home health, oncology or pain and symptom management.
• Current CPR certification.
• Must have and maintain a valid driver’s license with a good driving record.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Assess physical, psychosocial, and spiritual needs of patient and family.
• Monitor and manage pain and symptoms; educate patient and family.
• Coordinate Plan of Care with transdisciplinary team.
• Serve as liaison to physicians and communicate patient needs.
