Job summary

Reserve Care is seeking a dedicated Registered Nurse (RN) to provide prescribed medical treatment and personal care to terminally ill patients, collaborating with a transdisciplinary team. You will coordinate care plans, assess needs, monitor symptoms, and support patients, families, and volunteers.

The role requires an Ohio RN license, current CPR, a valid driver’s license, and experience in hospice care or related fields.

Qualifications

• Licensed RN in Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license and a good driving record.
• Hospice/Palliative Care Certification desired; must be obtained within first 2 years of hire.
• Experience preferred in hospice care, home health, oncology or pain and symptom management.

Responsibilities

• Provide prescribed medical treatment and personal care services to terminally ill patients in collaboration with the multidisciplinary team.
• Coordinate Plan of Care implementation using the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Monitor pain and symptoms; educate patients and families on management.
• Liaise with physicians; maintain documentation and plans of care.
• Offer emotional support to patients and families; coordinate with volunteers and other staff.
• Participate in care conferences and quality improvement activities.
• Assist with discharge planning and on-call rotation as scheduled
