Job summary

Reserve Care in West Campus, Fairview Park, OH is hiring a Home Care Registered Nurse (RN) for a full-time role. You will provide prescribed medical treatment and personal care to terminally ill patients in coordination with a transdisciplinary team and implement the Plan of Care using the Nursing Process.

Responsibilities include assessing needs, managing pain and symptoms, supporting patients and families, and participating in care conferences.

Qualifications

• Licensed RN in Ohio.
• Current CPR certification.
• Valid driver’s license and auto insurance acceptable to RC.
• Hospice/palliative care certification desired; required within first 2 years of hire.

Responsibilities

• Provide intermittent skilled nursing services to terminally ill patients.
• Assess physical, psychosocial, and spiritual needs with the team.
• Coordinate Plan of Care using the Nursing Process.
• Participate in care conferences and bereavement follow-up.
• On-call rotation and discharge planning as assigned.
