Job summary

Reserve Care is seeking a licensed Registered Nurse in Ohio to provide prescribed medical treatment and personal care to terminally ill patients in collaboration with a transdisciplinary team. The role supports the Plan of Care across multiple settings and requires active participation in care conferences and discharge planning.

You will assess needs, monitor pain, provide emotional support, and coordinate with physicians, staff, and volunteers.

Qualifications

• Licensed Registered Nurse in the State of Ohio.
• Current CPR certification required.
• Valid driver’s license with good driving record.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Assess physical, psychosocial, and spiritual needs with the care team.
• Monitor pain and symptoms and educate patients and families.
• Participate in care conferences and discharge planning.
• Assist with coordination of the Plan of Care across disciplines.
