Job summary

BAYADA Home Health Care is seeking a Hospice RN to deliver compassionate home-based care to clients within Bucks County, Montgomery County, and Northeast Philadelphia. This role involves on-call duties, night visits, and coordinating with a multidisciplinary team to ensure high-quality palliative care at home.

Applicants should hold an active PA RN license with at least one year of clinical experience; home health or hospice experience is preferred. Travel to cases is required.

Qualifications

• Active and current license to practice as RN in PA.
• One year of clinical experience preferred.
• Home health or hospice experience welcomed.
• Ability to travel to cases as assigned.
• Compassionate, communicative, and reliable care.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to the manager.
• Follow up with doctors' orders and document appropriately.
• Occasional on-call after hours and/or weekends.
