BAYADA Home Health Care is seeking a dedicated Hospice RN for independent home visits in the Hatboro/Bucks/Montgomery/Northeast Philadelphia area. You will join a multidisciplinary team to deliver compassionate palliative care, with after-hours on-call coverage as needed and a focus on safety, documentation, and patient comfort.

Ideal candidates will hold a PA RN license, have hospice or home health experience, and demonstrate strong communication and organizational skills.

Qualifications

• Active PA RN license required.
• Experience in home health care or hospice is preferred.
• Strong communication and assessment skills essential.

Responsibilities

• Make home visits to patients in designated territories.
• Administer medications and provide wound care and comfort measures.
• Monitor patient conditions and report changes to the care team.
• Document orders and coordinate with on-call and weekend coverage.
