Job summary

Providence at Home with Compassus seeks a Hospice Registered Nurse to deliver compassionate, patient-centered nursing care in home settings. You will assess needs, develop and direct individualized plans with the interdisciplinary group, and triage visits remotely while performing field visits during evening, night, and overnight shifts.

You will collaborate with IDG, administer medications as prescribed, participate in on-call duties, and ensure thorough documentation and ongoing professional

Qualifications

• Associate or Bachelor’s degree in Nursing from an accredited program.
• Current, active RN license in state of employment.
• BLS certification within 30 days of hire.

Responsibilities

• Assess patient and family needs and determine Hospice requirements.
• Provide professional nursing care using the nursing process.
• Develop and direct Plan of Care with the IDG.
• Reevaluate patient status and revise Plan of Care as needed.
• Administer medications and treatments per Plan of Care.
• Participate in on-call duties and maintain productivity per guidelines.
• Document accurate clinical notes and care provided.
