Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to assess patient and family needs, develop and direct individualized care plans, and deliver compassionate home-based hospice services.

You will collaborate with the Interdisciplinary Group, provide nursing care using the full nursing process, and support patients and families during end-of-life care, including evenings, nights, and overnight shifts as needed.

Qualifications

• Required: Associate or Bachelor's degree in Nursing.
• RN license current and unencumbered.
• Within 30 days of hire: BLS certification (American Heart Association).
• Preferred: 1 year Hospice experience; CNA/LPN hospice considered.

Responsibilities

• Complete patient/family assessments to determine hospice needs.
• Write and initiate Plan of Care with IDG collaboration.
• Regularly reevaluate needs and revise Plan of Care.
• Participate in on-call duties and maintain productivity.
• Document clinical notes and supervise staff when required.
