Job summary

Elevation Hospice of Colorado is seeking a Registered Nurse to provide skilled nursing care in patients' homes, coordinating with the interdisciplinary team to ensure high-quality hospice services. The role emphasizes patient- and family-centered care, education, and collaboration across care providers.

The ideal candidate will hold an RN license, have 2+ years in clinical care, and be proficient with computer systems/EMR.

Qualifications

Graduate of an accredited Associate or bachelor’s degree in Nursing.

Responsibilities

Administer skilled nursing care for clients in their residence and coordinate with the interdisciplinary team.
Perform initial and ongoing assessments of physical, psychosocial and environmental needs and ADLs.
Educate patients and families about disease, end-of-life care, and self-care techniques; train staff as needed.
Initiate preventive and rehabilitative nursing procedures; document progress.
Coordinate all patient/family services with IDT and refer to other services as needed.
Inform physicians of changes in patient needs and outcomes; adjust care as required.
Determine scope and frequency of services based on acuity and needs; assess caregiver ability.
Execute on-call duties and provide on-call support as assigned.
