Job summary

Optimal Home Care is seeking a Registered Nurse to provide in-home patient care in Colorado. The RN will perform clinical visits, care planning, and coordination with the care team to ensure high-quality services for our patients.

The role includes supervising LPNs, completing SOC/ROC/RECERTs, and collaborating with marketers and hospice staff to support transitions and resource referrals. A valid Colorado RN license and at least one year of RN experience are required.

Qualifications

• Graduate of an accredited, approved school of professional nursing.
• Active Colorado nursing license in good standing.
• Minimum of 1 year RN experience; home health a plus.

Responsibilities

• Complete SOC/ROC/RECERTs, supervisory visits, and revisits as needed.
• Complete documentation per agency and Medicare guidelines.
• Provide RN care within scope of practice; supervise LPNs.
• Coordinate care across accounts/territory and attend resident reviews.
• Collaborate with marketers and hospice staff; support transitions and resources.
• Fiscally manage patient cases and prepare fiscal reports as requested.
