Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to provide compassionate hospice care for our patients in Colorado. You will perform comprehensive assessments, develop and revise care plans, coordinate with the interdisciplinary team, manage pain and symptoms, and educate patients and families.

A current RN license, CPR certification, and a valid driver's license are required; hospice experience is preferred.

Qualifications

Registered nurse with current licensure to practice professional nursing in the state.
Graduate of NLN-accredited school of nursing.
Maintains a current CPR certification.
Current valid driver’s license.
Hospice experience preferred.

Responsibilities

Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
Provides professional nursing care by utilizing all elements of the nursing process.
Assesses and evaluates patient’s status; writes and initiates plan of care; revises as needed.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals based on nursing diagnosis and includes patient and family.
Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
Counsels patient and family in meeting nursing needs and self-care.
Provides health care instructions and supports activities of daily living.
Acts as Case Manager coordinating patient care for assigned caseload.
Records and communicates changes in condition; coordinates with physician.
Maintains continuity of care with family and interdisciplinary team.
