Job summary

Elevation Hospice of Colorado in Littleton is seeking a registered nurse to provide skilled nursing care for clients in their homes. You will coordinate care with the interdisciplinary team and referring agencies, and take responsibility for overall care coordination.

The role includes assessing patient needs, implementing and updating the care plan, and training staff as needed. You will support patients and families through end-of-life care with compassion and professionalism.

Qualifications

• Nursing degree from an accredited program (ASN or BSN).
• Current RN license in the state.
• CPR certification active.
• Minimum two years RN experience in clinical care.
• Hospice or home health experience preferred.
• Proficient computer use; MS Office skills preferred.
• Strong interpersonal, communication, and organizational abilities.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs.
• Implement the individualized plan of care and revise as needed.
• Educate patients/families about disease process and end-of-life care.
• Provide training to other staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes showing progress toward goals.
• Coordinate services with the IDT and referring agencies.
• Determine scope and frequency of care based on acuity.
