Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years' RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient with computer usage; Microsoft Office preferred.

Responsibilities

• Initial and ongoing assessments of patient needs and ADLs.
• Coordinate care with the IDT and families.
• Educate patients/families about disease process and end-of-life care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing care goals progress.
• Coordinate services and prioritize needs with IDT.
