Job summary

Elevation Hospice of Colorado is seeking a compassionate Registered Nurse to administer skilled nursing care in patients' homes and coordinate care with the interdisciplinary team. The role emphasizes patient- and family-centered care within the hospice framework.

Two years of RN experience in a clinical setting are preferred, along with CPR certification and a valid RN license. Strong communication and organizational skills are essential to delivering high-quality end-of-life care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' needs and ADLs.
• Implement the individualized plan of care and revisions as needed.
• Coordinate care with the interdisciplinary team and referring agencies.
• Provide on-call responsibilities and support to patients’ families.
