Job summary

Advanced Home Health & Hospice of Colorado Springs is seeking a Hospice RN Case Manager to coordinate and manage the care of patients in a hospice setting. You will provide skilled nursing services and oversee care plan implementation.

The role requires strong clinical knowledge, excellent communication, and the ability to work autonomously while collaborating with an interdisciplinary team to honor patient wishes and deliver compassionate end-of-life care.

Qualifications

• Current RN license in the state of practice.
• 2 years of RN hospice or related healthcare experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of regulatory requirements and guidelines governing hospice care and documentation.
• Experience with electronic medical record systems and documentation preferred.

Responsibilities

• Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
• Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
• Coordinate and supervise the activities of hospice aides and other healthcare professionals involved in patient care.
• Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
• Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
• Collaborate with the interdisciplinary team to develop and implement patient care strategies that align with patients' wishes and goals.
• Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
• Provide bereavement support and resources to patients' families and caregivers as needed.
• Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
• Perform all other duties assigned by the Hospice Director or designated supervisor.
