Job summary

Elevation Hospice of Colorado is seeking a Registered Nurse to provide skilled nursing care for hospice patients in their homes. The role includes coordinating with the interdisciplinary team and referring agencies, while managing end-of-life care and patient comfort.

Responsibilities include assessments, care-plan implementation, patient/family education, on-call coverage, and collaboration with colleagues to ensure high-quality, compassionate care.

Qualifications

• RN with an accredited Associate or Bachelors in Nursing.
• Current state RN license and valid driver's license.
• CPR certification required.
• Two years RN experience in clinical care; hospice/home health preferred.
• Proficient computer skills; MS Office experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients’ needs and ADLs.
• Implement and update the care plan (POC) as needed.
• Educate patients and families about disease, end-of-life care, and self-care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Coordinate care with the Interdisciplinary Team and referrals.
