Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to provide comprehensive hospice services in Colorado. The role involves patient assessments, care planning, and coordination with an interdisciplinary team to ensure high-quality end-of-life care.

The RN will deliver nursing care per the physician's plan, educate patients and families, and coordinate services while maintaining clear documentation and ongoing communication with the care team.

Qualifications

• Registered nurse with current licensure to practice in the state (Colorado).
• Graduate of National League for Nursing accredited school of nursing.

Responsibilities

• Completes initial, comprehensive and ongoing patient assessments and families to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and implements a care plan with patient and family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Coordinates patient care as Case Manager with the Clinical Supervisor.
• Maintains accurate clinical notes and communicates with physicians and the team.
