Job summary

Three Oaks Hospice, Inc. is seeking a registered nurse to administer skilled nursing care at patients' homes, coordinating with the interdisciplinary team and families. You will assess needs, implement the care plan, educate families, and ensure high-quality end-of-life care.

Responsibilities include documentation, on-call duties, and collaboration with physicians and caregivers to adjust care based on acuity and patient/family needs.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing.
• Current state RN license and valid driver's license.
• Two years RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal, ethical and communication skills.

Responsibilities

• Administer skilled nursing care for clients in residence; coordinate with IDT and referring agency.
• Perform initial and ongoing comprehensive assessments of patients' needs and ADLs.
• Implement and revise the Plan of Care as needed.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Provide training to other staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes documenting progress toward goals.
• Coordinate services and prioritize needs with IDT members.
• Use case management approach and refer to other services as needed.
• Inform physicians of changes and evaluate response to care.
• Determine scope and frequency of services based on acuity and family needs.
• Assess caregiver ability to meet patient needs; adapt care accordingly.
• Communicate information using current organization processes and systems.
• Apply admission and re-certification criteria for hospice care.
• Perform on-call responsibilities and provide on-call service as assigned.
• Other duties as assigned.
