Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to administer skilled nursing care in patients' homes and coordinate with the interdisciplinary team for high-quality end-of-life care.

The role emphasizes thorough assessments, care planning, education for patients and families, and on-call coverage as needed. Strong communication and ethics are essential to support patients and caregivers.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience; hospice/home health preferred.
• Current CPR certification and proficient computer skills.

Responsibilities

• Administer skilled nursing care in clients' residences and coordinate with the IDT.
• Perform comprehensive assessments and develop/update the plan of care.
• Educate patients/families on disease process, end-of-life care, and self-care.
• Provide on-call nursing coverage as assigned.
• Document care and communicate changes to physicians and team.
