Job summary

Three Oaks Hospice is seeking an experienced Registered Nurse to administer skilled nursing care for patients in their homes. The role centers on coordinating care with the interdisciplinary team, patients, families, and referring agencies to ensure high-quality hospice services.

The ideal candidate will hold an Associate or Bachelor’s in Nursing, be a licensed RN with CPR certification, and bring strong communication and organizational skills to a patient-centered environment.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal and communication skills; high ethical standards.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Perform on-call responsibilities as assigned.
• Document clinical progress notes and care plans.
• Assess patient and caregiver needs and adjust care plan as needed.
• Facilitate admission criteria and re-certification for hospice care.
