Job summary

Reserve Care is seeking a Registered Nurse (RN) to join our on-site hospice team in Ashtabula, OH. The RN will provide compassionate, patient-centered care across the care continuum, from assessment to coordination with the interdisciplinary team.

Responsibilities include direct clinical care, phone follow-ups, and documentation that aligns with our standards. Must hold an Ohio RN license and have hospice experience for best fit.

Qualifications

• Active Registered Nurse license in Ohio.
• Minimum two years of hospice/palliative care experience preferred.
• Strong written and verbal communication with patients and families.

Responsibilities

• Provide direct nursing care using the nursing process for patients with complex end-of-life needs.
• Collaborate with interdisciplinary team and primary physician to plan and adjust care.
• Perform telephone follow-ups and triage calls to ensure patient comfort and safety.
• Document care activities clearly in RC's guidelines.
