Job summary

Miracle City Hospice in Ashtabula, OH is seeking a compassionate RN Case Manager to provide primary clinical support for patients and families navigating hospice care. You will manage caseloads, conduct assessments, and coordinate with the interdisciplinary team to deliver individualized, compassionate end-of-life care.

You will visit patients regularly, educate families, and ensure timely documentation while supervising home health aides and participating in ongoing quality-assurance

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current RN license in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assurance and patient care standards.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Managing a patient caseload from admission through ongoing care and evaluation.
• Completing comprehensive assessments and developing individualized care plans.
• Collaborating with physicians and the interdisciplinary care team to coordinate patient-centered care.
• Providing regular patient visits and updating care plans as needs change.
• Supporting patients and families with education, guidance, and compassionate care.
• Maintaining accurate and timely clinical documentation.
• Supervising and coordinating care provided by Home Health Aides.
• Providing end-of-life support and clinical pronouncement when necessary.
