Hillcrest Hospice Care is seeking an experienced Clinical Care Coordinator (RN) to lead the hospice clinical team. You will blend direct patient care, team coaching, and care coordination to ensure high-quality end-of-life services.

You will collaborate with the Director of Clinical Services, supervise staff, and participate in essential training, regulatory compliance, and on-call rotation. This role emphasizes leadership, clinical excellence, and compassionate patient-family support.

Qualifications

• Graduate of an accredited nursing program required.
• Nebraska or compact-state RN license required (valid).
• Bachelor’s degree in Nursing preferred.
• At least two years of RN experience in a hospice setting required.
• At least two years of leadership experience in a hospice setting preferred.
• American Heart Association Basic Life Support (BLS) certification required.
• Valid driver’s license and reliable transportation required.

Responsabilités

• Lead and support the hospice care team in daily operations.
• Coordinate referrals and review potential hospice patients.
• Assign patients and coordinate clinical coverage across shifts.
• Provide patient visits and admission coverage as needed.
• Conduct comprehensive nursing assessments for direct patient care.
• Coordinate services across the interdisciplinary hospice team.
• Educate patients and families on disease progression and end-of-life care.
• Triage calls from team members, patients, and families.
• Oversee onboarding and ongoing education for clinical staff.
• Participate in quality assurance and improvement activities.
• Assist with payroll oversight and clinical staffing needs.
• Build relationships with internal and external partners.
